About Primary Care Behavioral Health at CHCW
The practice of primary care behavioral health model resembles and looks a lot like primary care and working with your PCP. Behavioral health consultants (BHCs) will form meaningful relationships with you and your family for your entire life. They are embedded within the medical team and are easily accessible. Also, they help with anything and everything, as well as all ages that the clinic services. The BHC team at CHCW try to make behavioral health care as routine and as accessible as possible.
We Help With Everything
At CHCW, we look at behavioral health as anything that relates to behavior. This could include concerns such as depression, anxiety, trauma, and also include helping people improve their diet, sleep, and physical activity. Additionally, people trying to change behavior, such as quit smoking or start a new medication or exercise routine, BHCs are able to help come up with effective plans to ensure success!
The Importance of Integrated Behavioral Health
Integrated care involves full collaboration with your Primary Care Physician (PCP). In this model, behavioral health consultants (BHC’s) and providers jointly plan and execute goals, develop integrated care plans, co-manage patients, and maintain shared schedules.
The main goal of the integrated care program is to improve communication between behavioral health and primary care providers and thereby improve care coordination. We aim to:
- Increase availability of behavioral health services within the medical model
- Provide support to the PCP in addressing patients’ behavioral health needs
- Mitigate negative impacts on physical health
- Improve patient clinical outcomes and increase overall satisfaction with care through the integrated care model.
